PLEASE FILL OUT EACH PACKET IN ITS ENTIRETY IN PEN.  ALL FORMS ARE REQUIRED TO BE COMPLETED AND MAILED IN ORDER TO BE CLEARED THROUGH THE SPORTS MEDICINE DEPARTMENT.
